Re: the F-15 over Benghazi.... In the world of GA (General Aviation), that kind of accident is actually common, and almost always attributed to the pilot: he/she is in charge of deciding whether to fly the plane and if it's improperly weighted, that's his problem.
If the crash was caused by the plane being improperly weighted, that sounds like pilot error to me. I'm only a humble single-engine piston pilot, but if my plane crashed because it was flying while improperly loaded, the FAA would blame ME as the pilot for that accident.
