Former SAR pilot here - it can definitely happen. When there's a crash and you're scrambled and reports and questions are coming in over the radio and your co-pilot is trying to DF on the ejection seat tone and the guys in back are calling out sightings - the adrenaline gets pumping, and mistakes get made. It's just the nature of SAR, particularly within units that only get called out to actual rescues infrequently.
