Actually I cannot get behind The Big Conspiracy theory. It's fun to speculate and hypothesize that it's a sinister plot instead of careless management, but if you are dealing with governmental workers, the oxymoron is obvious. Yes, the space program has had it's share of bumps (please read "A Man On The Moon" by Andrew Chaikin) and human errors are very visible when you are trying to control a multi-megaton detonation to achieve earth orbit. Sadly it's just engineers not checking their figures, or managers providing insufficient oversight. Too bad.
