Thanks, mate. In the Nuclear Community where I work, we trend precursors to accidents, to try to isolate behaviors or knowledge-based deficiencies before they lead to errors. When an organization or business acts to correct a issue only after an event happens of a magnitude that causes significant capital damage, or puts someones' safety at risk or worse, then it shows the lack of attention to detail by the leaders. The guys in the blue suits who let O'Grady fly were just as responsible for him losing a F-16 as he was. His errors continued after he was shot down BTW, when the Marines landed the Sea Stallion to extract his lame ass, he came charging out of the brush, gun in hand. If it had been a CSAR Operation run by the guys that usually do it he would have been justifiably shredded by a minigun. He knew that he was supposed to sit there with his ID card in his hands over his head. He didn't do that either. To the Air Forces' credit I think they pulled his ticket. Anyway thanks for the off-topic discussion.
