The problem with the dirty bomb is the number killed immediately could be zero. If you look at the nuclear dispersal tests done at the NTS near the TTR, the contaminated areas are quite small. There isn't much contaimination due to radioactivity in the air from a dirty bomb, but rather the radioactive particles attach themselves to the dirt.

So you set off a bomb and maybe there are 1500 additional cancer cases in 10 years. I suspect that won't be nearly as effective as a suicide bomber at a shopping mall.
