To add to Peter's excellent post, the Air Force Instruction for unit emblems is AFI 84-105; google it if you want to read the full-bore bureaucratese involved. Many of the patches produced in connection w/programs are not unit emblems, per se--unit emblems have to meet all the requirements that Peter mentioned, including final design and registration by the Army's Institute of Heraldry--but instead are called 'morale patches.' They're intended for informal use, e.g., they can be worn on flight suits, but not on other AF uniforms.
