Normally, for private requesters FOIA is free for up to 2 hours of search time (doesn't apply here anyway), and 100 pages of document copies. (The other day, I actually received ~150 pages of copies from AFHRA for free.)

The only explanation I have, is that the map (for whatever reason) is not subject to FOIA -or- there is some "small print" in the FOIA law and regulations, which says reproduction costs for "unusual" formats (like maps) apply to every requester. In case of the latter, however, a total fee of $12 is rather low, and in my experience fees below $15 are waived by default (IIRC, it actually says so in a USAF FOIA regulation). So my guess is that the map falls outside FOIA - maybe it's regarded as a commercial product and not a government document.
