TKM appears in the flight plans. There are often BE20 flights from TNX to TKM. As far as I know, all 737 flights claim TNX as the destination. It is only the BE20 flights that use TKM. You can catch the TKM airport code on passur.com when the BE20's fly IFR.

You are correct that TKM does not appear in the ICAO list. Technically, it would be KTKM. I would assume 4 letter airport code for Tikal would not start with a K.
